How to set up my first appointment
Step 1
Our email is HIPAA secure so please email us to begin therapy @ Laila@RisingPhoenixPLLC.Com
Step 2
We will email you a secure link back requesting the following information:
- Name
- Date of Birth
- Mobile Number
- Email Address
Step 3
You’ll receive a welcome email with a personalized link to complete your e-paperwork. We utilize TherapyNotes as our HIPAA portal for patients.
Step 4
Once your e-paperwork is completed you’ll be scheduled for an intake appointment.
